CHA Patient-Directed Care Collaborative Webinar Series - Part 4 Location: Live Webinar, Presented at your Facility

Please join the Patient-Directed Care Collaborative, an interactive webinar series, which will be held monthly from May 2017 – April 2018. The Institute of Medicine (IOM) has declared that patient-centered care is a hallmark of high-quality care. Through the Patient-Directed Care Collaborative, participants will learn best practice strategies to advance patient-centered care delivery and a culture which sustains it. Utilizing this 12-month collaborative approach, teams from a variety of hospitals will work with each other and our faculty to identify process changes that will lead to lasting improvement in patient and family engagement. The collaborative will meet virtually on the third Tuesday of each month from 12:00 p.m. – 1:00 p.m. from May, 2017 through April, 2018. There is no cost to attend but pre-registration for each session is required. Featured will be Tiffany Christensen, a nationally recognized public speaker and the author of three books exploring advocacy, end of life planning and partnership strategies in healthcare. Born with cystic fibrosis, Ms. Christensen has received two life-saving double lung transplants and today uses her patient background and professional training to address a variety of key topics revolving around the patient perspective. The series is being offered under the Hospital Improvement Innovation Network (HIIN) a CMS national initiative aimed at advancing patient and family engagement and reducing events of preventable patient harm.
