Symposium: Reducing Heart Failure Readmissions

Healthcare professionals from all disciplines are encouraged to attend this program, which will also serve as a kick-off event for the 2010 statewide collaborative—Reducing Heart Failure Readmissions.
Every year, nearly 5 million people in the United States experience heart failure, which can significantly affect a person’s ability to function in daily life, and it is the leading cause of hospitalization and readmission among older Americans.
There is strong evidence that heart failure re-hospitalizations can be significantly reduced. The collaborative will help participating organizations standardize processes related to:
• providing evidence-based care to patients with heart failure;
• enhancing the admission assessment for post-discharge needs;
• engaging patients and their families as active partners in care;
• patient and family-centered hand-off communication;
• medication reconciliation, adherence, and safety;
• and post-acute care follow-up.

Getting to the Heart of Conflict - Skills for Healthy Conflict Management

It is clear to almost everyone that conflict is inevitable in life—in our personal lives as well as in the workplace. Different personalities, different work styles, cultural/ethnic norms, and differences in generational mix, all lead to an endless possibility of conflict surfacing at work.
What is not so clear, is the role conflict plays in the process of change and effective team problem solving—both
major factors in improving organization performance. How can we recognize and manage the sources and trigger points of conflict? When is conflict healthy—what makes it destructive? How can we reduce or defuse
unnecessary conflict? What are the various styles of dealing with conflict, and the risks and benefits of each
approach?
Following her successful program last November: From Difficult to Dynamic—Engaging Challenging Employees, Joan Evans will return to CHA to present this full-day, highly interactive program, frequently requested by supervisors and managers attending prior management development programs.

HIPAA Privacy: Rules of the Road

For members who were unable to attend CHA’s HIPAA programs last October and December, we will again
offer a half-day session focused on the HIPAA Privacy rule, but inclusive of the intersection with the Security
rule, including the latest enforcement actions and guidelines from both state and federal regulators. The
program will focus on strategies for maintaining continuous compliance, a review of the breach detection and
reporting process, a review of all ARRA—HITECH Privacy rules including new obligations for business
associates and how to update business associate agreements, a road map of what to expect next from the federal
government, and practical operating tips and solutions for remaining compliant with the Privacy and Security
rules as the transition from paper to EHR accelerates.

Optimizing Point-of-Service Revenue

The inability to accurately estimate a patient’s financial responsibility prior to service or discharge is a major
barrier to preventing revenue loss for hospitals. By closely studying hospitals from a cross-section of sizes,
demographics, and geographic regions, The Advisory Board will discuss how leading institutions use best
practice approaches to optimize their point-of-service processes to increase revenues, reduce bad debt, and
decrease the likelihood of collection and its inherent costs. The session will also include a product presentation
that offers a real-time solution to effectively manage the revenue cycle.

This year’s Patient Safety Summit, sponsored by CHA and Qualidigm, will examine several timely topics and
multi-faceted aspects of leading quality improvement and patient safety initiatives. Nationally recognized experts will focus on strategies for helping organizations measure the quality of care provided and if the changes made are improving the safety of the care processes, provide a first-hand account of the journey Duke University Medical Center has gone through to improve patient safety and transparency, the practices of high reliability organizations, and the value of patient and family-centered care programs. The program will be held at CHA in Wallingford.
